Liability Waiver

By submitting my registration application to Leap of Faith Studios, I hereby agree to the following: I waive, release, and discharge Leap of Faith Studios and its owners, instructors, employees, and associates of any and all liability and responsibility for injuries, sickness, accidents, and natural disasters incurred during participation in dance classes, rehearsals, performances, and during travel to and from such events. I agree to hold harmless the entities or persons mentioned in this waiver from any and all liabilities or claims made as a result of participation in this activity, and I certify that I have read this document, fully understand its content, and that I sign it of my own free will.
I give permission for my child to be photographed and/or videoed for the purpose of studio promotion, understanding that their likeness may be published but no personal information will be released.
